A player release form must be completed by each player on a team intending to participate in the AIHA. This is true for youth as well as adult and women team members. The PLAYER RELEASE FORM not only releases the AIHA from liability but provides vital information in case of injury as well as documented date of birth.

Please include team name, coaches name, home rink and the AAU CLUB CODE as indicated in the upper right corner of the release form (in red). The completed player release forms must be sbmitted to the AIHA by no later than the qualifier registration deadline of the team’s first participating qualifier.

The release forms have been formatted so that you may enter information into the underlined and boxed areas that require text entry. If your computer has the Adobe Acrobat program, you may enter your data and save the document with your data. If your computer has Adobe Acrobat Reader only, you may enter your data, however, you may not save it. Instead you will be limited to printing out a hard copy only.
