PLAYER RELEASE FORM

Each high school team must submit a PLAYER RELEASE FORM for every rostered player that participates on their team.  This form must be completed and sent to the IHAAZ prior to a team participating in an IHAAZ qualifier.

TEAM ROSTER AND ENTRY FORM

A TEAM ROSTER AND ENTRY FORM must be submitted to the IHAAZ by a High school team prior to participating in an IHAAZ qualifier.  This form must not only include each player that is officially on the team’s roster, but must also include any new players being added to the team’s roster as well as any players that are being removed from a team’s roster.

ADDING NEW PLAYERS TO A ROSTER

Team roster additions must be submitted no less than one week prior to the qualifier that a high school teams intends to participate in.  Any high school team, which has the required minimum of 6 skaters and 1 goalie (7 total skaters), may request to add a maximum of 2 players who are students at another high school.  The eligibility requirement for players being added to a high school team that are not students of that high school school is that their school district boundry must be adjacent to the the school district of the high school team they intend to play on.   This request must be submitted to the IHAAZ board for approval no later than one week prior to the qualifier.  A player draft form must be completed and submitted to the IHAAZ for review for each roster addition if the player or players are being added after a high school’s first qualifier.  Player/roster additions may not be conducted at a weekend tournament.  After a high school team’s first weekend qualifier, any player(s) being added to a high school team roster will incur a $15.00 fee to the IHAAZ as long as player information, AAU registration and high school affiliation is confirmed.

A high school team that has more than one team representing their school may make one player move from one team to another during the regular season.  Any player wanting to make a second move back to another team or their original team will need to get approval from the IHAAZ.
