PLAYER RELEASE FORM

Each youth & junior team must submit a one time PLAYER RELEASE FORM for every rostered player that participates on their team for the season.  This form must be completed and sent to the IHAAZ prior to a team participating in an IHAAZ qualifier.

TEAM ROSTER AND ENTRY FORM

A TEAM ROSTER AND ENTRY FORM must be submitted to the IHAAZ by a youth or junior team prior to participating in an IHAAZ qualifier.  This form must not only include each player that is officially on the team’s roster, but must also include any new players being added to the team’s roster as well as any players that are being removed from a team’s roster.

ADDING NEW PLAYERS TO A ROSTER

Teams wanting to add new players to their roster once a team has already played in a qualifier, will be required to submit a $15.00 fee to the IHAAZ for every new player added to their team’s official roster.  Fees are not required for teams who delete players from their roster.
